Patient Information

Patients are seen by appointment only, we will work urgent/emergency patients into the schedule as quickly as possible as schedule allows. As a courtesy, please inform the office 925-328-0255 or email staff@caldermcare.com to cancel or reschedule your appointment at least 24 hours prior to your appointment.

For New patients, please arrive 20 minutes prior to your scheduled appointment time to complete the registration process. The insurance and medical history form can be filled out online by login in to MyPatientVisit Portal (please refer to Patient Login page)” in addition to the Financial and Office Policy form prior to your visit.

If you desire a complete skin exam please let the receptionist know so that we can schedule appropriately. The wait for complete skin exam appointments may be longer than if you are just coming in to check a specific area of concern.

Our Patients do come first and this attention can sometimes cause delays in our schedule as emergencies or unusual situations may arise. We ask you to be understanding and recognize that this same attention and courtesy would be extended to you as our patient. Please tell us if timing is critical to your schedule on a particular day, and we will do our best to accommodate you.

As a courtesy, our clinic utilizes appointment reminder system send out text/phone/email reminders prior to the appointment date. Please confirm the appointment when you receive the reminder call.

We ship our entire portfolio of fine skin care products for existing patients.

Product Returns Policy: Your satisfaction is our utmost concern. If you are not satisfied with one of your products, please bring in the product with your receipt within one month of the purchase. We will gladly exchange returned product for a product or products that are more suited to your desired result. If you do not wish to have a product exchanged, there will be a 5% restocking fee deducted from the total purchase, a refund will be issued in the form of your original purchase. All cash and check refunds will issue within the 5 business days.
