Patients are seen by appointment only, we will work urgent/emergency patents into the schedule as quickly as possible As a courtesy, please call our office 925-328-0255 within 24 hours to cancel or reschedule appointments.

For New patients, please arrive 10-15 minutes prior to your appointment time, as it will be necessary to fill out patient information form, general office consent, and insurance paperwork prior to your visit. Please call office for either a user name and password or a 'security code' to create user name and password to log in and enter demographic, insurance information and medical history. In addition, please print out, sign, and bring new patient forms via download link below. If you are able to enter your demographics and medical history online, you may skip the first page of the downloaded patient forms; if not, please complete the entire patient forms.
